Here are rebased patches, along with the following change: there is now an
environment variable, `SAGE_PARALLEL_DOCBUILD`, which if set to anything
nonempty which doesn't start with "n", causes the reference manual to be
built in parallel. I also added "doc-parallel" and "doc-pdf-parallel"
targets to the main Makefile with a patch to the root repo.
