I added a bunch of little patches. If you'd rather, I can fold them all
in together, but I think it might be easier for you to look at them this
way. I tried to make each patch deal with one issue (though the first one
includes several). I think the doctests pass properly if you apply any
initial subsequence of the list of patches.
I am still working, but I thought you might like to see where I'd got to.
