I cannot see a prompt for hostname where I can put the name of the SQL server.
 
Under the DB check information I say that it is an SQL Server check, the database name (which is admn), the username and password and the SQL library to use.

hostname should be the name of the sql server
database name the name of the database.

The appropriate SQL libraries are selected. The username sa and password have been re-entered several times and are definatly correct.
 
SQL Server 200 is using SQL authorisation.
 
I have tried running the check whilst SA is running as a service and as an application.
 
How should I enter the database name, I have tried entering the name on its own and I have tried preceding the database with the name of the server but none of them work.

Did you specify a username/password for the check?
What authentication is SQL using?
