Smells very much like a bug I've was hitting in OpenLDAP.  I had index
corruption (specifically the objectClass index) in ldbm on OpenLDAP <
about 2.0.10.  I ended up moving to the latest 2.1, and also moved to
'bdb' - which is considered a more stable back-end.
