One you do something really bad in a transaction, you have to get a new one.
(Really bad is some kind of db exception.) Just restart the process to get a
new connection. (You can also use the low level transaction tools to get a
new transaction, but that it more trouble than is usually necessary.)

This exception is just evidence that something else has already gone wrong.
