My company (42 Coffee Cups, http://42coffeecups.com/talk/) spends
quite big time developing and supporting satchmo-based solutions for
our clients across the Europe, UK, and US.

Generally - it depends.  Mostly it is quite straightforward.

Sometimes we did have to branch off the satchmo codebase and give a
client a customized version.

We have done such project in late 2008 for Iceland media company.  I
can't recall something be very different from the usual setup.
