I definitely got it to work on Sandbox Paypal, albeit after some hardships.
I forget if I had to modify the code, but I seem to remember a good portion fo the code being completed when the site receieved a "notification" as opposed to when the customer came back to the site. My first check would be to make sure you have the test accounts set up properly. that was my first error.
