Recently, Savannah switched from Apache to nginx; this was an omission in the
new configuration, should be fixed now.

I guess you used some 'go back' mechanism of the browser; when the request
passes successfully, all attachment fields are cleared (and a warning is
issued)---we fundamentally can't add them to the preview.
