Hello,
some files say "(See accompanying file LICENSE_1_0.txt..." but this is out of
sync with the real filename.
Are the examples part of the distributed package? Then they should bear the
copyright+license notice too. For images, I usually recommend to put an
explicit text file about their license in the same directory, but as it is
just one, you might as well write a line about this in the README.
I trust you to fix this before uploading and can approve your project now.

Maybe you can write manually a notice to the main file of the documentation
like "This document is a direct derivate of the GPL'd source code and
therefore is subject to the same copyright&license, namely: ..."
