#348: Use base output class for src/io outputs

The base output class provides nice mechanisms (start/stop) and factorizes
some code (checking for infallibility, dealing with fallibility by
start/stopping). It would be nice to use it in src/io outputs -- at least
I'd like to know why it isn't used.
