Unfortunately, I don't have any log right now about the issue.
However, the crash was caused by a file being removed between
the last playlist reload and the time it was scheduled to be
played. I believe that it was prepared because I was using
playlist.safe at the time.
However I'm interested in achieving fault tolerance for all
situations you mentioned, including the deletion of a file
that's currently playing. My users have FTP access to their
music folder, so they're free to add/remove files and the radio
should never stop whenever they delete something.
