So, does this trauma cause permanent memory loss, or do the memories return in time? Each incarnation still thinks of himself as the same Doctor?

I was surprised to see him go not quietly into the night. When he was yelling "I'm not ready!" it was poignant. What exactly is the reason for this regeneration (outside the needs of the producers to keep a show running for decades). How different is the new Doctor each time?
