Summary: Hypertext parser hangs

  This occurs when a comment is found in strange looking SGML. The
lexer tries to style backwards and that causes failure. This case
should be looked at by someone who understands SGML to work out what
should be done properly in this case. For now I've patched it by
detecting the backwards movement for this particular case and avoiding
the ColourTo call.

  Changed DocumentAccessor::ColourTo to assert if asked to go
backwards. It used to just print a warning and continue, so this
should make such issues more visible in debug builds. For release
builds, it no longer tries to performs backwards styling.
