This removes visible keyword use in $SRC/tools
The keywords in the manpages were used for the date in the .TH.
In codereview.1 this was done via a macro that (I believe), was
crafted such that it could use either RCS or SCCS keywords (see also
the crafty use of '$Date: %E%$'. I removed that macro.
The keywords were expanded by hand, copyright unchanged.
The version of cw was expanded by hand, copyright unchanged.
I removed the use of WEBREV_UPDATED from webrev (the date presented at
the bottom of your index pages). I'm not yet convinced that's the
right thing to have done, reasoned opinion is solicited on both that
and cw.
