maybe you are using a *very* old version of ppm, or the 64bit version, which doesn't have a GUI. i installed ActiveState perl around the end of 2007, and it was like that already. but since the 64bit version lacks many modules (or at least it did at the time), i assume people are better off with the 32 bit version anyway.
