Depends how cheap is `cheap'. UK Solutions offer a `shell
account' for 85 UKL inc VAT which gives you everything you asked for
except the 2 Gb/month throughput (which would cost extra). They offer a
web hosting service for about half of that, with ftp-only access. In both
cases they allow you complete control of incoming mail addressses. They
fix problems _very_ fast. You can also run background processes with the
shell account, if you wish (primary target for the account was IRC server
folk).
I've used UK Linux too. They used to be cheaper than UK Solutions
but recently put their prices up. You don't get ssh access, but you can
dial in. I can't comment on their web hosting as I gave up trying to do
anything serious after discovering that I could never upload
email-forwarding files to the mail server and that the admins mostly
didn't answer email about problems (though when they do answer they are
quite helpful). I expect you get a much better service as a dial-in
customer.
I don't have any connection with either of these except as a very
satisfied/dis-satisfied customer repsectively.
