I don't have any macros or scripts, but my printer (Lightning Source) provides templates. Just import the template as an EPS or TIF, and lay out the elements as required. For whatever reason, Scribus produces huge PDF files in comparison to to InDesign: 20 megabytes vs. 4-5 megabytes. But that's not a big problem, as the PDF quality is excellent. I haven't yet tried Scribus for the interior of books.
