I'm using 1.5.5, and it puts “/PageLayout /TwoColumnRight” into exported
PDF. In my tests, most PDF viewers obey the instruction, including Adobe
Reader. Some still display the first page on the left. Some ignore and
use their own preference, however, — there probably isn't much a PDF
authoring application can do here…

Would it not be simpler to export your PDF as normal and use an app like
Bookletimposer or other PDF layout tool to do your layout for the customer.
