I've been a contractor for about a year, here's what I use:

Legal structures:
  Sole Proprietor, registered DBA using LegalZoom.com

Health Insurance:
  Kaiser, cheapest plans, no prescriptions drugs. About $175/mo.

Taxes/Invoicing:
  TurboTax/ Freshbooks. I've used accountants in the past but last
year I did taxes myself using TurboTax. Unless someone convinces me
otherwise: accountants seem to ask me all the same questions that
TurboTax does and then put it in some software. So with TurboTax I get
some nice web software instead of a long email chain to an accountant.
Freshbook is indespensible.

Collections for unpaid invoices:
  Local Mafia goons.
