Both of my thoughts on the subject are probably wrong given what you have said so far. I was thinking browser caching (but you won't see the old file under the new filename). And my other thought was needing to add the file to attr_accessible - but if you had a problem there, then your initial file upload wouldn't be working either. Do an edit on a development server and check the logs to see if you see information about the file data being rewritten - not just the metadata. -- Cynthia N.
