Basically you just need to copy/paste from either Servlet or Faces.
Here's what you need:
<xi:include xmlns:xi="http://www.w3.org/2001/XInclude"; href="book_info.xml"

<toc />
<xi:include xmlns:xi="http://www.w3.org/2001/XInclude";
href="introduction.xml"/>
...
And the introduction.xml should be:
<preface id="intro">

<para>...</para>
</preface>
