No one else has joined this bug, but it has continued to make life with
my expensive monitor extremely annoying.

And despite suggestions above, I am certain this is not a hardware
problem.

Here is new proof:
 yesterday, I initiated an upgrade (do-release-upgrade) to 22.04 from 21.10 
from  the command line.
By chance, I tried full-screening a video while the upgrade was in progress. 
Amazingly, it worked flawlessly the entire duration of the upgrade! ! ! 
 As soon as it was done and rebooted, I tried playing that video again and it 
did the black-screen / device searching every 3 seconds or so, as it always has.

So, how can I reproduce the thing that the upgrade did?! It said
something at the beginning about disabling screen lock or something like
that, but I have tried turning off all such things in my settings
already.

You mentioned "intermediate adapters". I'm using a Lenovo Ultradock between my 
laptop and the monitor.
 This causes no trouble on my Acer B326HK, which is just as high-res, but the 
new Samsung monitor continues to do the blackout thing. It's not strictly 
reproducible so far (fullscreening does not always cause any problem, and is 
not required to cause the problem).

Happily, I have two ultradocks.  It happens with both! And the brand new
HDMI cable looks fine.

I cannot try without the ultradock because my laptop does not have an
HDMI port.

No one else has reported this problem, though if it's a hardware fault
with the monitor (my interpretation) then it might be rare.  I have
recently upgraded to 21.04 and Wayland.

I am wondering:
Should I buy an adapter from HDMI to VGA, if such a thing exist?

You would be generous to give this any more attention. Thank you!

(Recently,) it only happens when I make something full-screen on the external 
monitor. I do not think this is a hardware (which is brand new) problem.
Just now I made a browser full screen and it immediately strted happening every 
several seconds for a couple of seconds at a time. I've attached the journal

Can't tell if it's the same problem, but a recent (<1 month) update on a
Lenovo T450s running 20.10 has led me to these symptoms: closing the lid
or calling suspend leaves the machine dark and unusuable until I hold
down the power button for 10 seconds to reboot.

Bug description:
  Whenever Ubuntu gets into suspend state, I cannot wake it up. 
  -Keystrokes start the fans but screen does not turn on and Ubuntu still seems 
to be suspended because in my teamviewer account the computer is still offline.
  -Power button clicks do not help
  -I always have to turn off computer by holding power button and then turn it 
on again.

  The only kernel that works is the following: "linux-image-5.0.0-1070
  -oem-osp1". But with this old kernel I run into different problems.

  I have the latest BIOS version from Dell (released 2020-11-27) and the
  latest updates in Ubuntu 20.04.

  I will post logs during suspend and important logs (found in Logs gui
  tool)

I'm on a Lenovo T450s and have this problem immediately after updating
to 16.10.  I believe I'm running 4.8.0-34-generic kernel (standard
default repos) and the problem still exists.  So I'm without a
workaround. If there is a fix other than default update, can someone
spell it out?

  Sometimes, when moving cursor from one monitor to the other, the
  external monitor turns black and gets back on again.

  The hardware is Dell Latitude E5470 with Docking Station and monitor
  connected via DisplayPort (checked both ports and happens on both of
  them).

  Everything worked correctly on Ubuntu 16.04, but it happens on Ubuntu
  16.10. Kernel 4.8.0-25-generic.

  The bug isn't related to cursor only, if focus changes by alt+tab it

  Problems with shut down and boot (Lenovo T450s)

Bug description:
  My Lenovo T450s under Ubuntu 15.10 does not poweroff normally.
  Instead, it hangs on "Reached target shutdown".  (See image)
  Along the way, I believe there is briefly a full page of 
"drm:gen8_irq_handler [i915] *ERROR* The master control interrupt lied (SDE)!" 
messages.

  If I leave it there for a long time (> 10 minutes?) it does/may
  actually turn off without me forcing it off, but this makes no
  difference to the subsequent behaviour.  Moreover, if I press Ctrl-
  Alt-Del fast, many times, I get the "Rebooting immediately" message,
  but it doesn't.

  When I try to boot, I experience the following. It requires at least
  two reboots to be able to log in:

  On the first boot, the Grub page is skipped, and it hangs on the Ubuntu logo 
with moving dots.
  Eventually, I reboot it. 
  The second time (this is all reproducible), it boots but very slowly. It 
stalls at "britty.service" and eventually gives me a message
  about dev-disk-by , where it waits for 90 seconds before booting.

  After that, things proceed to a normal login screen. Behaviour after that is 
sometimes normal.
  This is a 1 month-old  computer.

Ah. Pressing Ctrl-Alt-F1 does display some shutdown status messages,
though they get strangely duplicated if I flip back and forth.

In any case, shutdown gets to:

[Ok] Stopped D-Bus System Message Bus

and then hangs.

  My Lenovo T450s under Ubuntu 15.10 does not poweroff normally.
  Instead, it hangs on "Reached target shutdown".  (See image)
  Along the way, I believe there is briefly a full page of 
"drm:gen8_irq_handler [i915] *ERROR* The master control interrupt lied (SDE)!" 
messages.

  If I leave it there for a long time (> 10 minutes?) it does/may
  actually turn off without me forcing it off, but this makes no
  difference to the subsequent behaviour.  Moreover, if I press Ctrl-
  Alt-Del fast, many times, I get the "Rebooting immediately" message,
  but it doesn't.

  When I try to boot, I experience the following. It requires at least
  two reboots to be able to log in:

  On the first boot, the Grub page is skipped, and it hangs on the Ubuntu logo 
with moving dots.
  Eventually, I reboot it. 
  The second time (this is all reproducible), it boots but very slowly. It 
stalls at "britty.service" and eventually gives me a message
  about dev-disk-by , where it waits for 90 seconds before booting.

  After that, things proceed to a normal login screen. Behaviour after that is 
sometimes normal.
  This is a 1 month-old  computer.

Update: I take that back, or its reverted: the machine will NOT shut
down and, in fact worse than before, after the upgrade there seems to be
no way to see the terminal output during shutdown.  Pressing ESC during
shutdown now gives a blank screen rather than showing what is going
wrong.

The machine just keeps showing moving coloured dots on its shutdown
screen forever, until I force a power down by holding the power button.

I have upgraded to 16.04.  There are new problems with RAM/swap
(https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1585598) but
shutdown, which didn't work once in recent months on 15.10, works as it
should now under 16.04

I've just upgraded to 16.04 from 15.10, and now the memory usage
steadily increases from when I first boot up. After just an hour or two,
all 20GB of RAM is used up, even if nothing is running, and I am forced
to reboot (rebooting frequently is incompatible with my normal tasks).

much.  I understand from free that it is all buffered/cached?

Both htop and free suggest that the memory is really not available.

  I've just upgraded to 16.04 from 15.10, and now the memory usage
  steadily increases from when I first boot up. After just an hour or
  two, all 20GB of RAM is used up, even if nothing is running, and I am
  forced to reboot (rebooting frequently is incompatible with my normal
  tasks).

No one else has mentioned this symptom:

While left idle, memory usage steadily creeps up even without anything going on.
In a couple of hours or less, htop shows 16GB of RAM in use (and still slowly 
climbing), even though the applications, sorted by RAM usage, don't seem to 
account for that much.
 I have tried shutting down everything optional I can see (dropbox, chrome, 
...), with no change.

I have a series of symptoms of horrid problems with this laptop, so I am going 
to mention more of them in case they're related. Whenever I undock the laptop 
from my ultradock, unity-sessions-daemon crashes and either then or some other 
time, my swap space accumulates 1.2 GB or so of use, even though I have 20GB of 
RAM and it is mostly not in use.
After, this various other things often don't work and I have to reboot, which 
leads to the problems filed above. 

For instance, right now the swap still shows some usage, which it
shouldn't, and choosing to shut down the computer from the taskbar  does
nothing.

  but it doesn't.

  When I try to boot, I experience the following. It requires at least
  two reboots to be able to log in:

  On the first boot, the Grub page is skipped, and it hangs on the Ubuntu logo 
with moving dots.
  Eventually, I reboot it. 
  The second time (this is all reproducible), it boots but very slowly. It 
stalls at "britty.service" and eventually gives me a message
  about dev-disk-by , where it waits for 90 seconds before booting.

  After that, things proceed to a normal login screen. Behaviour after that is 
sometimes normal.
  This is a 1 month-old  computer.

My Lenovo T450s under Ubuntu 15.10 does not poweroff normally.
Instead, it hangs on "Reached target shutdown".  (See image)
Along the way, I believe there is briefly a full page of "drm:gen8_irq_handler 
[i915] *ERROR* The master control interrupt lied (SDE)!" messages.

If I leave it there for a long time (> 10 minutes?) it does/may actually
turn off without me forcing it off, but this makes no difference to the
subsequent behaviour.  Moreover, if I press Ctrl-Alt-Del fast, many
times, I get the "Rebooting immediately" message, but it doesn't.

When I try to boot, I experience the following. It requires at least two
reboots to be able to log in:

On the first boot, the Grub page is skipped, and it hangs on the Ubuntu logo 
with moving dots.
Eventually, I reboot it. 
The second time (this is all reproducible), it boots but very slowly. It stalls 
at "britty.service" and eventually gives me a message
about dev-disk-by , where it waits for 90 seconds before booting.

After that, things proceed to a normal login screen. Behaviour after that is 
sometimes normal.
This is a 1 month-old  computer.

earlier similar bug for Chrome was fixed for 14.10.

I log in using Guest Login from the lightdm startup screen.

I launch chromium-browser or google-chrome.

Instead of a window appearing, the terminal says, after some seconds

Check failed: NamespaceUtils::DenySetgroups()

and then does nothing.

fresh installation of Ubuntu 15.04, it does not give you Montreal.

  I just installed a new 14.04 server install on the following hardware:

  ASRock E3C224-4L Server/Workstation Board (Intel C224 chipset)
  Intel Xeon E3-1245V3 Quad Core LGA1150 CPU
  32GB of DDR3 ECC RAM
  LSI 9201-16i SAS/SATA HBA

  I am unable to power down the system without physically doing it with
  the button, or powering it off using the inbuild IPMI management.

  All of the following result in a reboot:

  sudo shutdown -h now
  sudo shutdown -p now
  sudo shutdown -P now
  sudo halt now
  sudo halt -fihp now
  sudo init 0

  I have tried acpi=force, acpi=off, acpi=noirq, noapic

  I tried the script from
  http://askubuntu.com/questions/13288...-of-a-shutdown as follows:

  That didn't help.

  I also set WOL off on the NIC that was plugged in (only one of the 4
  onboard NICs is connected at present, for testing) with sudo ethtool
  -s eth0 wol d and it didn't help, I've looked in the BIOS, and can't
  find a WOL section in there. To try to eliminate WOL, I also tried
  with neither the NIC in use for testing, nor the dedicated IPMI NIC
  connected, and it didn't help.

  I've updated my BIOS to the latest version.

  I've probably tried more that I've forgotten to list here - I've
  exhausted my options!

  I'm at a loose end now - no idea what to do next to further
  troubleshoot it, has anyone else had any similar problems on C224, or
  does anyone know what logs I should be inspecting, or how I can
  generate further debug logs?

I had this problem a lot on a Lenovo X230Table under 14.04 (and maybe the 
previous distro) but the latest firmware update (BIOS update) from Lenovo seems 
to have fixed it.
I have not yet tried upgrading to 14.10.

(Incidentally, if I'm allowed to say so here, my machine still fails to
shut down sometimes: that is, it reboots instead of shutting down, even
with the latest BIOS. This can be very bad due to the risk of
overheating if it's being put away...)

I tried the 3.14 kernel. 
 Under either kernel, the problem does not occur every time I shut down, 
unfortunately. I am not sure what the variable is.
In any case, it just did the same thing (restarted when I asked for a shutdown) 
under the upstream kernel.

Bug description:
  On Lenovo X230 Tablet, and the latest 14.04, when I pick Shutdown from my 
desktop menu, the machine shuts down and then almost immediately boots up 
again, by itself.
  Instead, it should shutdown and stay down.

  Because this can cause the hardware to overheat when I turn my machine
  off and put it in its bag, I suggest this is high priority for the
  LTS.

  (I've no idea whether "upstart" is the right package.

Hello. I am a naive user.
There seems to be a choice of files to install. I want the amd64 ones, but 
do I want lowlatency or generic?

If I isntall them, will it just look like another option in grub?

  On Lenovo X230 Tablet, and the latest 14.04, when I pick Shutdown from my 
desktop menu, the machine shuts down and then almost immediately boots up

Bug description:
  On Lenovo X230 Tablet, and the latest 14.04, when I pick Shutdown from my 
desktop menu, the machine shuts down and then almost immediately boots up 
again, by itself.
  Instead, it should shutdown and stay down.

  Because this can cause the hardware to overheat when I turn my machine
  off and put it in its bag, I suggest this is high priority for the
  LTS.

  (I've no idea whether "upstart" is the right package.
