The tooth in the eye

DR. SRINIVAS K RAO, DR. SUJATHA MOHAN, DR. MOHAN RAJAN

The tooth has more uses than chewing. It can also help restore vision in
some cases.
Photo: K.R. Deepak
I CAN SEE: A revolutionary procedure gives a priceless gift.
An eye for an eye only makes the world go blind, said Mahatma Gandhi.
But using the tooth can help restore vision in blind patients with
terminal corneal disease. This revolutionary procedure affords the
priceless gift of sight to patients otherwise destined to a lifetime of
darkness.

The technique

Originally described by Strampelli in Italy, it was modified extensively
by Giancarlo Falcinelli, his student, who termed the procedure modified
Osteo-Odonto Keratoprosthesis (MOOKP). He has perfected the technique
over the past 40 years and facilitated the transfer of knowledge -
allowing patients in India to benefit from the complex surgery - since
2003.

The term OOKP refers to the use of a bone-tooth (osteo-odonto) complex
to create a plastic cornea (keratoprosthesis). This composite is the
heart of this surgery and allows patients unsuitable for corneal
transplantation to see again. Since the plastic cylinder does not
require nutrition to remain clear and viable, this surgery can work even
in extremely dry eyes.

Such ocular conditions are encountered in persons who have suffered
extensive chemical burns to the eyes, severe drug reactions (termed
Stevens Johnson syndrome), immunological destruction of the ocular
surface, and in other conditions like poorly fitted contact lenses,
multiple ocular surgical procedures, and other genetic conditions.

In persons who have such disease in both eyes, vision can be limited to
light perception only, and they are unable to move around without help
and support. Since many of these conditions affect young adults in the
prime of their life, there are considerable economic and social
implications to this problem. The MOOKP offers help to such patients.

When assessing patients for suitability, the doctor will look at the
potential for vision, the state of the teeth and general health of the
patient. If found suitable, the surgery is performed in two stages with
a three-month interval between the two. It is a complex procedure
requiring eye surgeons, dentists, and skilled anaesthetists.

Different stages

The first stage is preparatory; the canine tooth is harvested and the
surface of the eye is prepared by removing scar tissue and is
reconstructed using a mucosal flap from the inside of the cheek. The
tooth is bonded to the plastic cylinder and the composite is placed in a
skin pocket under the eye to develop a connective tissue covering.

Three months later, the composite is removed from its pocket. The
mucosal flap on the eye is peeled back and a hole is made in the cornea.
The interior of the eye is prepared for the cylinder, after which the
composite lamina is placed with the cylinder inside the eye. The mucosal
flap is then replaced to protect the cylinder, which protrudes through
an opening in the mucosa.

Post-operative care

Post-operative care includes use of antibiotics to protect against
infection and regular dressings to ensure the health of the implant and
the mucosa. After a week of such care, the visual improvement can be
determined. Further care is less intensive and patients can return to
their normal activities thereafter.

Although the procedure can help restore vision in patients with advanced
ocular surface damage who cannot be helped by any other procedure, it
requires extensive surgery - often two stages, each lasting six hours or
so.

The post-operative appearance of the eye is unlike that of the normal
eye due to the use of the mucosal flap.

While central vision is often normal, the use of a plastic cylinder
restricts the field of vision to about 30 to 35 degrees. However, this
is quite enough to restore functional capabilities to such patients -
some of whom can even drive vehicles - albeit carefully. Long-term
results with this procedure have also proven quite encouraging.

There are many advances in medicine and ophthalmology, but the
resurgence of this technique in the past decade - due to the kindness of
the Italian surgeon and the endeavours of the Indian surgeons has helped
more than 100 Indian patients regain the gift of sight. The tooth thus
has uses other

KOLKATA: The completed works of India's Nobel Laureate poet Rabindranath
Tagore will now be available free online along with the notations of
songs that he composed.

The project, conceived and implemented by the State Information
Technology Department, was launched by Chief Minister Buddhadeb
Bhattacharjee earlier this week.

When told about a powerful search engine that will be part of the
project, Mr. Bhattacharjee enquired about certain songs of his choice,
which were then promptly searched for and displayed on a giant screen
put up for the launch.

As part of the 150th birthday celebrations of Gurudev, as Tagore was
reverentially called, the notations of all 2,200 songs written and
composed by him and numerous letters will be made available online.
Currently, some 100 songs and their notations will be available on the
site,

Bengal Engineering and Science University (BESU) helped with making the
entire project compatible with UNICODE - the global platform that
improves accessibility of the Bengali language for an international
viewer - the Indian Institute of Technology-Kharagpur helped create the
search engine which will allow the reader to locate a song, line or even
a film based on a Tagore novel.

Pranav,
A long shot: try an older version of jaws. I can see some links on Web pages
with jaws 10 that I cannot see with jaws 11 -- this apparently has to do
with some defaults related to graphics size settings being different between
the 2 versions of jaws.
Alternatively, you could try increasing the minimum and maximum graphics

[AI] Disability groups apprehensive of amendments to laws

2010-02-07 Thread Renuka Warrier

Disability groups apprehensive of amendments to laws

J. Venkatesan

NEW DELHI: Disability groups on Sunday expressed strong apprehension that the
Centre was going ahead with proposed amendments/harmonisation in various laws
without consulting them.

Participants at the two-day meet that concluded here on national
cross-disability consultation, titled 'Harmonisation of laws with the United
Nations Convention on Rights of Persons with Disabilities [UNCRPD],' felt that
the government would make a shoddy job of it and compromise the real objectives
if there was no effective consultation.

The Human Rights Law Network , together with the National Association of the
Deaf and Deaf Way, organised the event in order to formulate the views of all
the disability groups and submit its recommendations. It was pointed out that
India signed the UNCRPD on March 30, 2007, and ratified it on October 3 of the
same year. In corollary to this development, the Government of India is to
harmonise/amend various laws in the Budget session 2010 in sync with UNCRPD in
three disability-specific laws and in 75 other laws by May 3, 2010, and submit
a 'compliance report' to the United Nations.

The meet said despite the magnitude of harmonisation required, the Centre had
been sleeping for the last two years and did not seek any consultation with
disability groups .

Therefore, there is a strong impression within the disability sector that the
government is rushing with the amendments process to the Persons with
Disabilities Act 1995 in the budget session of Parliament and will do a shoddy
job, as is reflected in the draft amendments document posted on the website of
the Ministry of Social Justice, which does not include almost 20 Articles of
the convention and is completely silent on the civil and political rights of
persons with disabilities.

Re: [AI] query regarding external hard drive.

2010-02-07 Thread Arpit Jain
Hello!

The term dividing you are referring to is actually called partitioning
and this can be easily done using the windows inbuilt partitioning
utility or by using any third party applications such as partitioning
magic.
To go to windows inbuilt utility, just right click on my computer and
select manage. A new window will open up with the title computer
management. This window is divided into two parts - the left part
contains a tree view from which you can select different choices and
on the right side the options related to that choice will appear.
You need to select disk management from the left side and then carry
on the process on the right side..
Note: While using windows utility to partition, you need to empty your
drive, this is a bit of a drawback.

[AI] Feedback solicited - Angel eBook Reader.

2010-02-07 Thread Sudhir R (NeSTIT)
Hi folks !

Just came across a low-priced gadget called Angel which seems to offer
most features of Victor Reader Stream and such other devices that are
priced at least 3 times over.

Angel offers text reading, voice recording, music and daisy / audio book
playing, radio etc.

Would like to get user feedback from people who might have tried this
out, especially regarding :

* Quality of TTS used - is text clearly understandable as with
say, Eloquence TTS of Jaws ?

*Ease of navigation

*Quality of audio (for music listening).

*Ruggedness

*After sales support, etc.

Appreciate if experienced users could comment, especially with respect
to more established models like Victor, Plextalk etc.

Thanks and rgds

RS
