The hubble lens grinder off-ed himself due to his mistake. That was a pretty bad mistake. And some middle manager at NASA chose to not test the mirror prior to being installed. Super brilliant folks working on it. I once shipped 2000 units of a product that had a software glitch. Stack overflow after 32 operations. It was OTP ram based MCU. Expensive buggers. I tried so hard to find a bit to burn to change an op code to cause a jump to unused memory to vector around the problem. I totally failed. Send an employee with 2000 chips to change. I am wholly appreciate what they accomplished.
