I reproduced this bug with some difference with OOo 2.0.1 and Window XP
Professional Version 2002 Service Pack 2 and Microsoft Office Word
2003((11.6502.6408) sp1, Part of Microsoft Office Professional Edition 2003).
The followings are steps for reproducing:

1. Open OOo 2.0.1 writer, click on menu: View - Toolbars - Form Design, see
that the icons of form design group appear on bottom with only “Select” and
“Form Navigator” are bright. 

2.1 If you look at the three icons in “Form Design “line on the bottom: “Display
Grid”, “Snap to grid” and “Guides When Moving”, you will find they are 
clickable. 

2.2 Click on “Display Grid” icon, then, “Design model on/Off”, “open in design
model”, “Automatic focus” icons appear and enabled. 

2.3 If you keep “Display Grid”, “Snap to grid” and “Guides When Moving” icons
state unchanged and don’t click on them, but you click on Form Navigator” ,
“Design model on/Off”, “open in design model”, “Automatic focus” icons also
appear in the “Form Design “line and enabled.  


3.1 When you double click on “Form Navigator”, you will also get a popped up
small window with inside item “Form”; 

3.2 Then, you right click on “Form” to choose “New” in the small window, you get
a standard form;

3.3 Then, you can see” “Guides When Moving” icon is brightened and “Add field”
icon is enabled;

3.4 Click on new Form inside the small window several times, you can see “Form”
and “Activation Order” icons are enabled. 

4. I made further tests:
 
4.1 Click on the small rightward arrow besides “Guides When Moving” icon, then
click on appeared “visible buttons”, I find all above mentioned commands or
controls items have been already ticked. 

4.2 Then, I move mouse to “customize” that is below the “visible buttons”;

4.3 click on it, a “customize” window with “Toolbars” tab is pops up and there
is a label” Toolbar” on the window, the label’s corresponding  value is fixed as
“Form Design”;

4.4 then, click on “Add” on this dialog, another dialog window with multiple
command items pops up. 

4.5. The description in top of this window said “…Then drag the command to where
you want it on the desired toolbar… Under the description, there are 2 small
windows, one is ‘category”, another is “commands”. Each category on left window
correspond some commands in right side window. But I can’t drag any command to
the place on bottom line—Form Design tool bar where I want (This problem is
similar with the problem reported in “qa issue 59186”). 

4.6 However, I can drag those controls to  “Tool bar content commands” inside
the first “Customize” window from second “Add commands” window. And at same
time, the controls I want to add appear on the bottom Form design tool bar and
now they are good.  

5. I guess that the respond of user events and link among models may have some
problems.

2. If you look at the three icons in “Form Design“ toolbar on the bottom: you
will find 3 icons --“Display Grid”, “Snap to grid” and “Guides When Moving”--
are clickable. 

2.1 If click on “Display Grid” icon, then you will find another 3 icons ——
“Design model on/Off”, “open in design model” and “Automatic focus”——are 
enabled. 

2.2 If not click on “Display Grid”, “Snap to grid” or “Guides When Moving”
icons, but click on “Form Navigator” icon, then “Design model on/Off”, “open in
design model”, “Automatic focus” icons will also be enabled in the “Form Design“
toolbar.  

3. When you double click on “Form Navigator”, you will: 
3.1 Get a popped up small window with inside item “Form”;

Also, I did some further tests:
 
1 On “Form Design” toolbar, click on the small downward arrow besides “Guides
When Moving” icon, then move mouse to “visible buttons” on the drop-down menu, I
find all above mentioned commands or controls items have been already ticked in
the long appeared menu. 

2 Then, I move mouse to “Customize Toolbar” on the drop-down menu which is below
the “visible buttons”;

3 click on it, a “customize” window with “Toolbars” tab is popped up and there
is a label ”Toolbar” on the window, the label’s corresponding  value is fixed as
“Form Design”;

4 then, click on “Add” on this dialog, another dialog window with multiple
command items pops up. 

5. The description in top of this window said “…Then drag the command to where
you want it on the desired toolbar… Under the description, there are 2 small
boxes, one is ‘category”, another is “commands”. Each category on left side box
correspond some commands in right side box. But I can’t drag any command to the
place where I want on bottom “Form Design” toolbar (This problem is similar with
