I'm in the same case and you, and the D(digits) options of Dial command works
fine for me. I use it this way:

exten => _9.,1,Dial(SIP/[EMAIL PROTECTED]||D(w6969w${EXTEN}))

Where 6969 is my calling card number (or whatever).
