As it happened, I landed at Waikerie (in my Stemme - its flying again after a 
long hiatus), on the day after this happened (i.e. yesterday). I turned up 
there for reasons quite unrelated to the accident - and spoke to some of the 
people who had witnessed the accident (again, as a side effect of happening to 
be there anyway, nothing more).

So - as it happens, I do have some idea of the answer to your question… but I’m 
not going to speculate/explain here, because in doing so, I’d be communicating 
third hand information about an accident that is actively under investigation - 
which I would see as an irresponsible thing to do. 

This applies to any accident - I’m not singling out this one. 

We all have this totally understandable tendency to want to immediately know 
*how* such an accident happened - I’m sure we’re all motivated by the same 
thing here (trying to figure out how not to have it happen to us in the future, 
whatever ‘it’ is!).
