Has anyone done an implementation of UCCX and MediaSense with
AgentInfo? I'm using UCCX 10.6 and MediaSense 11.0

Yea, but you'll need to upload each separately. So Root and Intermediate
should be 2 separate files uploaded.

Is the MediaSense certificate signed by a CA or self-signed? You may need
to upload the whole chain if it's signed.

Yes, I added the .pem certificate MediaSense as tomcat -trust in UCCX, but
the problem still persist.

Do you have the Mediasense certificates as a tomcat-trust on both UCCX
servers? May just be a certificate issue.
