Apologies for not replying sooner.
We don't have our ILLiad request function set up as a GES that can be
edited via the GES configuration menu. It may take me a bit to unravel how
we set it up.  If you'd like we can talk on the phone about what we have
set up.

Preserve Digital Ohio

Custom WordPress Form

Project Introduction

This document is a Request for Information (RFI) regarding vendor
capabilities in creating a custom WordPress form for the Preserve Digital
Ohio (PDO) website. The PDO team is seeking a contractor to create a custom
form that will guide users through a questionnaire and insert
user-submitted responses into a spreadsheet template that the user will
then be able to download via the frontend, preferably in Microsoft Excel
format. The data will also be collected in a backend database. The
questionnaire should be optimized for usability.

Preserve Digital Ohio is a project team that aims to assist small cultural
heritage organizations in intellectual and digital preservation of their
born-digital and digitized collections. The PDO website will be launched in
early October and is a tool that will assist organizations in this effort.

On November 5 and 6, 2015, Code4lib YEG is hosting a new, free, and
technology-focused event: Code4lib Alberta Unconference 2015.

We are seeking speakers to present lightning talks or workshops that will
share information, insights, and expertise on a range of technologies and
innovative projects that are being implemented in Alberta's library
community.

Lightning talks will be 20 minutes in length and workshops will be up to 90
minutes in length.
