This bug was recently fixed, an update to Totem 2.18 will probably
happen soon.

The only clues I have found so far is the following messages printed
when the disc is reloading;
closing
closing
closing
closing
gnome-mount 0.6

This seems to suggest gnome-mount, but I'm not sure.

The only way I can reproduce this is by manually unmounting (not
ejecting) the DVD and pressing "Play Disc" in Totem. Is this the same
problem you're having?
