The author of gphoto2 (Marcus Meissner) has tracked down this problem:

He suggests that the "Continous AF" needs to be set to "Off" in the camera menus. On my Canon EOS 650D it was not only that, but also "AF method: Quick mode" will be needed. When this setting is selected, the "Continuous AF" is grayed out in the camera menu.

With the above two camera settings, I get both the coarse and fine focus working using darktable 1.6.6-2 and libgphoto2 2.5.7-5 and the following lenses:

EF-S 18-135mm 1:3.5-5.6 IS STM
EF-S 60mm 1:2.8 IS USM

It works equally well with the lens in the AF or MF mode.

The only focus-related setting in the Darktable GUI seems to be "focus mode: One Shot". I think that Darktable should either try to ensure that these extra settings are configured to sane values (so that the manual focus control works at all), or it should expose these settings in the GUI.
