Reopening, I just had this happen again, and I took a screenshot of it. You can see that the t contact is hidden (offline) in gmail chat but online in empathy. The volume becomes loud suddenly as soon as I touch the volume control in Totem (in the totem interface, not by going to sound preferences as in bug #411042). It also happens if I'm trying to lower the volume.
