To reproduce this problem in Lucid, create test.tex:

\documentclass[a4paper]{article}
\begin{document}
\begin{equation}
\vec{\nabla}
\end{equation}
\end{document}

Then:  latex test;dvipdf test;evince test.pdf

You will produce 4 or 5 lines of Error: Illegal entry in bfrange block
in ToUnicode CMap

Note that the bug occurs with both output produced by dvipdf and ps2pdf
(both ghostscript) but does not occur with pdflatex.
