I also have this problem (on kdm), ctrl + alt+ backspace doesn't work. I'm also 
using the fglrx driver so it might be a driver bug.
This bug also reports problems with the nvidia drivers:

This is exactly the same problem that I have with kdm. Sometimes it
works just fine to logout but sometimes the screen just turns black  and
CTRL + ALT + BACKSPACE doesn't work.

Does it happen if you edit /etc/X11/xorg.conf to use ati instead of
fglrx?

ctrl-alt-F1 doesn't respond anymore indeed.

Lately, I try not to use the logout functionality, to avoid this behavior. And 
I can recall when the last time was that I did try this. So it would take to 
long to search for. 
And as I just entered the office, I'm not going to try it now. I'm still in the 
process to convince them that it's ok to run Linux in the office. So invoking a 
crash wouldn't be a smart thing. I'll give it a try around noon (CET+1) and 
update the bugreport afterwards.

Tried the logout again. Apparently the behavior has changed a bit. No the 
screen just turns black. Still not possible to get into another console or do 
anything else. After a forced reboot, checked the messages and syslog, but 
nothing indicates anything being crashed.

Any specifics you want or do you want me to post the loggings themselves?

No, it's not useful after a restart. Do you mean that ctrl-alt-F1
doesn't work by example? Might be a linux or video driver issue, do you
have an issue logged to /var/log/messages or /var/log/syslog?
