  HP Pavillion desktop 2015 ufei date. Never turned off because boot takes too 
long.
  Screens (2) turned off system left on when not in use.
  Up to Ubuntu 19 and early versions of 20 both screens came back on.
  With latest update HDMI does not restart.
  If boot from power off both screens, HDMI and VGA are ok. Turn screens off 
and HDMI will not display.

  I note that Windows (horrible) has a switch that allows the HDMI to be
  controlled but Ubuntu did it automatically but won't now.

  Suspend has always killed the HDMI but there is not much difference in
  power consumption between on with suspend and on with screens off.

  Hope you can help, booting from full power off takes too long.

  HDMI not working properly

Bug description:
  HP Pavillion desktop 2015 ufei date. Never turned off because boot takes too 
long.
  Screens (2) turned off system left on when not in use.
  Up to Ubuntu 19 and early versions of 20 both screens came back on.
  With latest update HDMI does not restart.
  If boot from power off both screens, HDMI and VGA are ok. Turn screens off 
and HDMI will not display.

  I note that Windows (horrible) has a switch that allows the HDMI to be
  controlled but Ubuntu did it automatically but won't now.

  Suspend has always killed the HDMI but there is not much difference in
  power consumption between on with suspend and on with screens off.

  Hope you can help, booting from full power off takes too long.

Thanks for the bug report but this is not a bug.

The login screen always exists separately on VT1 (Ctrl+Alt+F1) to your
desktop on VT2 (Ctrl+Alt+F2). And the desktop is not meant to lock
immediately. It should do after some period of time, but won't "if you
were listening a web radio on Firefox" because that's a normal means of
preventing the screen from locking.

  I don't know if it's worth it, but you'll know. 

  from working on the graphic mode, if I go to the text terminal mode
  using [Ctrl]+[alt]+[F3] login do some work or not, then want to go
  back to graphic mode with [Ctrl]+[alt]+[F1].

  Normally, I should be login to the graphic mode to go back, but if you
  use [Ctrl]+[alt]+[F2] instead, it bypasses de login screen. Tried it
  with both of my PC's, same thing.

  I tried it on X11 and Wayland, same thing.

  If we wait long enough to pass the preset time out of the graphic mode
  when on text terminal , and try to come back to graphic, it disables
  the bypass.

  But, if you were listening a web radio on Firefox in between, the
  bypass is still accessible way after the time out.

Obviously GRUB and YouTube shouldn't be sharing the same screen. That
looks like a driver/browser chroma-key bug. What browser were you using?

I'm also not totally confident that's even the main issue you were

  I suspect that this "application" came from a GNOME extension that

  Because I thought that it loaded AFTER the Activities overview opened
  at log in. This repeats every time I reboot or just log off and log
  back in.

  Steps to reproduce:
1. Update the Impish system to the latest version
2. Reboot

appear in the Activities overview

Confirmed. It's because desktop-icons-ng is actually an application
window. There are custom hacks in mutter to make it look like a desktop
window. Only Xorg sessions have proper native support for desktop
windows. So it looks like those custom hacks in mutter (and maybe gnome-
shell) need some work.

I mean the error at the top of this bug:

can only happen in Wayland sessions. So I assume this bug is about
Wayland sessions only. If that is incorrect then let us know.

Bug description:
  In Ubuntu 21.10 Impish Indri, I have done today's updates, including
  GNOME 40, and when I rebooted, I saw the desktop icons in the
  Activities overview that has the grey gears icon and the gibberish
  name. However, when I clicked on it, I'm taken to the desktop. When I
  open the Activities overview again, that "application" didn't show up.

  I suspect that this "application" came from a GNOME extension that
  handles desktop icons, called gnome-shell-extension-desktop-icons-ng.
  Because I thought that it loaded AFTER the Activities overview opened
  at log in. This repeats every time I reboot or just log off and log
  back in.

  Steps to reproduce:
1. Update the Impish system to the latest version
2. Reboot
3. Log in

  Expected results:
The "application" that came from the desktop icon shell extension doesn't 
appear in the Activities overview

  Actual results:
This "application" shows up in the overview with the grey gears icon and 
the gibberish name every time a user signs in to the new GNOME 40 desktop

Yes Ubuntu and GNOME sessions are really the same thing, just with
different themes.

My only suggestion would be to finish testing:

  sudo apt install unity-session
  sudo apt install xfce4-session
  sudo apt install cinnamon-session

so that we can rule in or out 'mutter' being the cause here, or to see
if it's a general graphics driver issue.

  If the trash is empty (has no deleted files), the trash bin wasn't
  open at the time, and I right-clicked on it in the dock, it would
  result in the broken empty menu that appears in the top left corner.
  Moreover, it always appears on top of every window and the top bar,
  essentially blocking the Activities button and some parts of the
  application button that shows the menu for the app.

It is intentional that the screen lock is inhibited (temporarily
disabled) when watching a video. That may happen if you're listening to
music too, and is probably intentional.

It sounds like you are requesting that your screen automatically lock
when you switch to a virtual terminal.

As a workaround, I suggest you lock your screen yourself when you want
it locked. There is a keyboard shortcut: Windows Key + L

  I don't know if it's worth it, but you'll know. 

  from working on the graphic mode, if I go to the text terminal mode
  using [Ctrl]+[alt]+[F3] login do some work or not, then want to go
  back to graphic mode with [Ctrl]+[alt]+[F1].

  Normally, I should be login to the graphic mode to go back, but if you
  use [Ctrl]+[alt]+[F2] instead, it bypasses de login screen. Tried it
  with both of my PC's, same thing.

  I tried it on X11 and Wayland, same thing.

  If we wait long enough to pass the preset time out of the graphic mode
  when on text terminal , and try to come back to graphic, it disables
  the bypass.

  But, if you were listening a web radio on Firefox in between, the
  bypass is still accessible way after the time out.

Twenty years later and this is still unresolved?

Going to System Settings > Input Devices > Keyboard > Advanced > Swap
ESC and Caps Lock is a piece of cake.

Then going to System Settings > Input Devices > Mouse there is nothing for 
changing e.g.
Wheel Down > Screen down 
Wheel Up > Screen up
Button Forward > Control + Left click
Button Back > Shift + Left click
