No, pavucontrol doesn't have the same issue, but it has something else:

HDMI / DisplayPort 3 (plugged in) shows output level correctly, but
Digital Output (S/PDIF) doesn't move output level at all. (I do switch
output in system settings).

In Ubuntu Unity 21.04 Beta xserver-xorg-video-intel is installed by
default, but doesn't cause artifacts. In a test version prior to Beta it
did cause them.

Regarding #25, I suggest removing it from the installer. I don't mind
archives. I reported it against Ubuntu Unity:

  I've updated to Ubuntu 20.04 a couple of weeks ago, and there are some
  minor (but pretty annoying) issues with Gnome Shell every time I load
  the system. The background image has some green/violet/black visual
  artifacts, and the text elements in Gnome Shell incorrectly display
  some of the symbol. Changing the background image removes the
  artifacts until the next reboot (but if I select the previous one
  without rebooting, then I still see the same artifacts, so it's
  probably cached somewhere). Restarting the shell via Alt+F2 `r`
  usually removes the artifacts from the background image, but the text
  issues are still present.

  The issues are reprodicable with and without extensions, with 1.0, 1.5
  and 2.0 display scaling factors applied, and they are not user-
  specific (i.e. I was able to reproduce them after creating a new user
  from scratch)

  As far as I can say, only Gnome Shell is affected. All the
  applications (including Gnome/GTK apps like Geary, Gedit, Tilix, both
  "native" and flatpak) work fine without any issues

This has been happening to me since I upgraded to 17.04 developer
preview. As soon as I run Ubuntu in VirtualBox (either 16.10 or 17.04
guest and 17.04 host) and redirect Android phone to it, the system goes
wild. Files in /var/log on the host grow to occupy the whole space, all
memory is also taken and the processor (at least one of the 8 threads)
goes 100%.

The situation does not happen when the phone is in PTP.

I haven't reproduced it so far in 16.10 developer preview. I agree with

  next power-up).

I managed to reproduce the problem right after fresh install, with no
extra packages. (Not on live USB though). On Kubuntu 16.04 the funny
wallpaper briefly appeared during start frow live USB and after
installing it on the partition, I could not set up two independent
displays at all.

On Mint Rosa 17.03 with KDE I had to reboot after I left two monitors
off for the night and kept the system running, but otherwise two
monitors have been working out of the box for the last six days.

@Shahbaz, in what ways Ubuntu was complaining? For me it wasn't
displaying any error messages with Unity 7, just behaving erratically.

Installing gnome-settings-daemon does not help. The presence of this
package does not affect the bug's occurrence:

  fonts in Nautilus. When this happens. Nautilus doesn't display menu in
  the Unity menu bar, but displays an additional button showing three
  horizontal lines for bringing up the menu.

  This problem has been with me for years, although sometimes disappears
  for months at a time when I run sudo apt-get update. This time it re-
  emerged after upgrading to 15.10 beta (Wily). It happens usually on
  power-up, and disappears after system restart. (To appear again on the
  next power-up).

It came back right after 16,04 was released, only this time it is worse:

1. The font is thin in the dash, menu, clock
2. Nautilus doesn't integrate with Unity
3. The left wallpaper is weird, as if it was trying to display two resolutions 
at the same time
4. There is no wallpaper on the right monitor
5. There are terrible artefacts on the right monitor if I move a window around

This happens around 2/3 of times that I boot.

I've reproduced it in 16.04, but it is no longer connected to Nautilus.
Both the clock and the Unity menu had thin fonts, Nautilus was okay.
(The same as comment #10). It happened after I had a power failure.

There are no held back packages when you format the system partition. I
did it at first, keeping files in /home, and even with all packages
removed/installed afresh, this didn't do it. Settings are being not
properly migrated from 15.10.

I nevertheless leave this bug as invalid, as it won't occur on a new
machine.

  I connected one 3440x1440 monitor (DP, primary, left side) and one
  Full HD monitor (HDMI, secondary, right side).

  Clock was displayed only on the left hand monitor, but clicking the
  settings button in the upper right corner of the left monitor would
  display the popup menu on the right monitor. It was hardly possible to
  hit anything with the mouse, as the cursor was moving correctly, but
  clicking would activate a button different from the one directly below
  it. Clock and the launcher would look correctly, but the windows were
  ugly and hardly legible.

  I had to move my windows to the very left edge of the left monitor to
  hit soft buttons correctly. It looks like the "clicked" coordinates
  were being translated to the overall width of the virtual desktop

Performing "do-release-upgrade -d" won't do the job. Only a fresh
install in which I specifically had to format the /home partition did it
for me. (I use the full HD monitor to watch videos in background).

I reopen this bug, because upgrading from 15.10 to 16.04 won't fix it
unless /home partition is formatted in the process.

  I connected one 3440x1440 monitor (DP, primary, left side) and one
  Full HD monitor (HDMI, secondary, right side).

  Clock was displayed only on the left hand monitor, but clicking the
  settings button in the upper right corner of the left monitor would
  display the popup menu on the right monitor. It was hardly possible to
  hit anything with the mouse, as the cursor was moving correctly, but
  clicking would activate a button different from the one directly below
  it. Clock and the launcher would look correctly, but the windows were
  ugly and hardly legible.

  I had to move my windows to the very left edge of the left monitor to
  hit soft buttons correctly. It looks like the "clicked" coordinates
  were being translated to the overall width of the virtual desktop

Yest, it is fixed in 16.04. Tested on Live CD.

Also, with the image you provided I am able to run resolutions over Full
HD inside a Virtual Box.

This helped only ever so slightly. Windows look only slightly better.
Clicked point is still not aligned with cursor's position. Windows are
still ugly, although a little bit more legible than before upgrading the
BIOS. I can easily perfrom simple opreations on the left monitor, such
as responding to an e-mail, but the right monitor is not usable due to
the misalignment.

Recently I had a bad USB hub. The computer had a problem with going past
the POST phase and apparently Ubuntu had a problem loading as well.
After it did load, the problem in question occurred EACH TIME, and
furthermore, mouse cursor would momentarily be replaced with an ugly
cross. Apparently the problem may be occurring when Ubuntu cannot
properly load due to other hardware failures. Three days after I
replaced the USB hub I haven't noticed the problem.

It's no longer relevant. Type: sudo add-apt-repository "deb
http://download.virtualbox.org/virtualbox/debian wily contrib" and then
install package virtualbox-5.0. It's version 5.10 and apparently it
doesn't require libvpx1. (I don't have libvpx1 in my system, and still
been able to install virtualbox-5.0 from repository).

EXPECTED:

A. Font in Nautilus should be the same as in System Settings.
B. The Extra button in Nautilus should not be there.

It affects me on HD 4000, Ubuntu 14.10 with latest updates. Is there a
workaround I could use? Does #130, #120 or #109 apply?

I am experiencing this situation virtually every time when VirtualBox
works in full screen mode on the RIGHT monitor. (Dual monitor setup).

I have used Christopher's solution from #62, but it didn't fix it for
me.

Although the original bug has been fixed, the screen blinks terribly if
the window is not active. The path is:

Open Windows 7 guest system in VirtualBox, full screen, on the right monitor.
It displays correctly in full screen mode.
Click anywhere on the LEFT screen, e.g. in Firefox.
Each time you move your mouse, or press a keyboard button, areas of the screen 
in the right monitor flicker.

Even after you close the VM, blinking artifacts remain on the right
screen. Turning off  3d acceleration in the VM does not affect the
issue.

I have tried VirtualBox-4.3.17-96212-Linux_amd64.run (both with Oracle
and extension pack and Guest Additions installed) with no luck. The
screen is still flickering. It is not 100% reproducible, but happens
very often.

I have seen a corresponding ticket in Oracle (#13410), but for
conveniecne I am reporting it here.

I had restarted the host machine again, and tested the test version of
VirtualBox 4.3.17 for the following two hours on a number of different
VMs, and havn't experienced the nasty flicker. (Ubuntu 14.10). It's been
apparently fixed.  For reference, I use Intel HD 4000 as my graphic
card.

Btw, VirtualboX 4.3.17 doesn't integrate its menu into Ubuntu Unity.

The mini toolbar in VirtualBox 4.3.17 (full screen) looks beautiful. It
used to have an ugly shadow right after I installed 4.3.16 in Ubuntu
14.04.

Is is still not fixed on my machine. I use two full HD monitors on
Compiz 0.9.12 with Ubuntu 14.10. When I play VirtualbBox on 2nd (right)
monitor, its left edge is right in the middle of the physical screen.

I have downloaded all up to date packages from their corresponding
repositories and rebooted the machine.

one line:
env UBUNTU_MENUPROXY= eclipse
Notice the space bar after the '=' character. Do not use EXPORT etc, do not set 
environment variables, use this exact command provided that 'eclipse' is the

  Steps to reproduce:
  1. Run Eclipse.
  2. Move the cursor to the top of the screen.
  3. Click on any of the menu items (File, Edit, Navigate, ...)

  Expected: The content of the submenus show up

  Actual: Only the top-level headers seem to be available. Nothing
  happens when I click on them either. They don't show up in the HUD
  either.

  It worked fine on 13.04, so I'm not sure what changed.
