In my case it helps to restart the service lightdm (sudo service lightdm restart) - this works around 50% of the time, but in some cases the system freezes afterwards and a hard reset has to be performed. Same problem in 16.04 with "Intel(R) Core(TM) i3-3110M CPU @ 2.40GHz" Lenovo Laptop. While HDMI cable is connected, TV shows "no signal". I have had some kind of success on this front. In order to get Lubuntu to boot and install I added nomodeset and idle=nomwait in the linux parameters in grub. I found here http://ubuntuforums.org/showthread.php?t=2042773 that nomodeset is the thing that is preventing the HDMI from working. I tried it (removing nomodeset but keeping idle=nomwait) and the first time I managed to boot and have the HDMI working perfectly.
