Bug description:
  1. Format and label a target Ext4 partion using Ubuntu 12.04
  2. Install 64bit 12.10 OS using that target without reformatting it
  3. Shut down 
  4. Boot an alternate copy of Ubuntu
  5. Restart selecting the newly installed OS
  6. Login then shutdown
  6. Boot an alternate copy of Ubuntu
  7.Fsck the newly installed OS allowing corrections to be made

  Each time the the newly installed OS is executed and then shutdown,
  even if execution only consists of logging on, a subsequent fsck will
  FAIL.

  I used Acronis True Image Home 2013 to create an image of the newly
  installed 64-bit Ubuntu 12.10, so I can recreate the symptoms of Ext4
  filesystem corruption 100% of the time by restoring from the image,
  booting, logging on and shutting down.

Ok, here are two version of last-shutdown-lsof.

last-shutdown-lsof-available-unchecked is a clean shutdown with network-
manager 'available to all users' unchecked.

last-shutdown-lsof-available-checked is a unclean shutdown with network-
manager 'available to all users' checked.

Further to #45, if I check 'Available to all user' (usually causing an
unclean filesystem at shutdown) and then do 'sudo service network-
manager stop' and then 'killall dhclient' the filesystem will then go
down cleanly.

Clint, I have tried what you suggested (uncomment line 132 at
/etc/init.d/sendsig), and caused an unclean shutdown by checking
'Available to all users' for current NM connection. On reboot, there was
no mention of dhclient in /var/crash.

Hi Fransisco' I agree that latest kernel has improved the situation. It
is now possible to simple uncheck 'Available to all users' in NM to get
a clean shutdown. Beware if you create a new network connection, because
it defaults to 'Available to all users' checked. Before the latest
kernel, this solution wouldn't work for me.

Not directly, but I would assume so, as unchecking 'Available to all
users' still works with 3.5.0-25.

I have also tried your suggestion at #60, thanks. It works for me -- but
I have reverted to the simple 'uncheck' as there seems no downside in my
installations and any future fix should be with NM not the init system.

Oops, I was wrong at #47. Integration not as expected. I meant that
menus are working without the this bug but within the application window
and not integrated into unity. Star Man Aquarius at #49 is correct. (You
can't expect TDF to provide unity integration for Ubuntu.)

This bug should be generalised to other file systems. It occurs using
xfs and jfs, as well as ext4. In my experience the fs is not relevant,
except some recover from an unclean shutdown better than others. (It's
easy to try different file systems using fsarchiver, don't forget to
change fstab to the new fs before reboot.)

Does anyone else see two lines about 'hub_port_status failed error
(-110)'  just before shutdown and immediately following 'mount: / is
busy'? These errors always occur on several computers using different
filesystems. (None of the workarounds mentioned above or in bug #1061639
resolve the problem of fsck/log replay on next boot.)

Is there a way to kill all usb processes before shutdown to try and
determine if the usb system is interfering with the clean unmount of the
filesystem?

I'm using kernel 3.5.0-23-generic #35-Ubuntu SMP Thu Jan 24 13:05:29 UTC
2013 i686 i686 i686 GNU/Linux

This may need a separate bug report, if the usb system is a possible
cause and other filesystems also run fsck/log replay.

Bug description:
  When using LibreOffice with Ubuntu 12.10 64 bit, the menus will stop
  working. That is, the menu is showing at the top of the screen, and
  you can select a menu item, however nothing happens. Once the behavior
  starts, none of the menu items work, including File Exit. The only way
  to get the menus to work again, is to quit Writer and restart the
  program. This is with NVidia driver...not sure if it happens with
  other video cards.

  This is with libreoffice version = 1:3.6.2~rc2-0ubuntu4 and
  indicator-appmenu 12.10.3-0ubuntu2.1 (currently in quantal-proposed)
  ONLY. If you do not have these installed, you are hitting bug 1064962
  or bug 1075263 resp..

I've worked around the dropout-then-mandatory-reboot problem in Oneiric
by using wvdial (NM still handles connection to router and on forwarding
to other devices). With wvdial, dropouts are reconnected and internet
access continues.

It's hard to measure but dropout frequency also seem less (this
obviously depends on many hard-to-control variables: weather, cell
range/load/usage, inside/outside etc).

Strange that I seem to have this problem and it is not being reported by
other mobile broadband users. (ISP is Virgin Mobile Broadband leasing
capacity on the Singtel-owned Optus network in Australia.)

Any hints in my logs as to the cause?

This is a kinda annoying bug in 11.10. If my connection drops-out
(fairly regular event), I can't reconnect without a reboot. I have tried
sudo killall modem-manager (worked in 11.04), disable networking and
sudo restart network-manager without effect. Am I missing something? Is
there an easy workaround? Can I downgrade network manager or other
package?

I tried sudo restart modemmanager and that seems to work after waiting
30 seconds or so.

I wonder, though, if there is a dropout, why does this process not get
automatically triggered if auto connection is set in network manager?

quicker to simply reboot to get a connect to the web. I have also tried

and get my network going again.

I'm not sure what logs would help debug this. Advice would be

Simple workaround seems to be 'sudo service network-manager stop'  'sudo
service network-manager start' whenever the modem drops-out. I've
created a script that can be used from a remote desktop to restart the
network.

Solution at #20 only works sometimes, maybe the problem is related to
the usb modem firmware 'freezing' or 'stalling', as the only way to re-
establish internet connection seems to be to withdraw the modem and
reinsert it. (I have recently upgraded firmware but the problem
remains.)

I am trying two separate suggestions on that assumption: George Vita's
suggestions for a usb patch cable

device node reset described by roman10 (http://www.roman10.net/how-to-
reset-usb-device-in-linux/).

It could still be a problem, but I have changed my method of connecting
to the internet. I now use a portable ZTE pocket wifi modem. So my
laptop now connects through wifi and not using the broadband modem --
the ultimate workaround (after trying all of the above). I now using
12.10 and dropouts are rare after a local network upgrade.

Alexander, get rid of NM and install wicd to learn if the problem(s)

  1. Format and label a target Ext4 partion using Ubuntu 12.04
  2. Install 64bit 12.10 OS using that target without reformatting it
  3. Shut down 
  4. Boot an alternate copy of Ubuntu
  5. Restart selecting the newly installed OS
  6. Login then shutdown
  6. Boot an alternate copy of Ubuntu
  7.Fsck the newly installed OS allowing corrections to be made

  Each time the the newly installed OS is executed and then shutdown,
  even if execution only consists of logging on, a subsequent fsck will
  FAIL.

  I used Acronis True Image Home 2013 to create an image of the newly
  installed 64-bit Ubuntu 12.10, so I can recreate the symptoms of Ext4
  filesystem corruption 100% of the time by restoring from the image,
  booting, logging on and shutting down.

Ernie, I'm still on 12.10 (I'm waiting for RC before upgrading) and
using wicd with a wireless connection, so I'm not sure why you are
getting an error with your ethernet connection. You have probably
explored all the options...

In the next couple of days I'll upgrade one computer to 13.04RC and
report back if wicd can be successfully installed or not. (It is
obviously in the 13.04 repos if you got it though Software centre.)

  1. Format and label a target Ext4 partion using Ubuntu 12.04
  2. Install 64bit 12.10 OS using that target without reformatting it
  3. Shut down 
  4. Boot an alternate copy of Ubuntu
  5. Restart selecting the newly installed OS
  6. Login then shutdown
  6. Boot an alternate copy of Ubuntu
  7.Fsck the newly installed OS allowing corrections to be made

  Each time the the newly installed OS is executed and then shutdown,
  even if execution only consists of logging on, a subsequent fsck will
  FAIL.

  I used Acronis True Image Home 2013 to create an image of the newly

during shutdown. This in turn triggers a bug in my BIOS which causes this 
system to hardlock in the middle of BIOS post when I reboot while still

Ernie, have you tried temporarily replacing NM with wicd on 13.04? (See
https://help.ubuntu.com/community/WICD)

When I use wicd, there is always a clean shutdown, without any delays.
On my 12.10 installation using NM 9.6.0, I can get clean shutdowns by
unchecking 'available to all users'. Checked, on the current network
connection results in an unclean shutdown -- 100% reproducible.

Max, I have not tested either. I am using the latest 12.10 version
(0.9.6.0-0ubuntu7).

I would be happy to test further. Is there a binary version available of
0.9.6.0 with the patch you mentioned?

BTW, my machine shuts down in 5-8 seconds using wicd, compared with
20-25 seconds with NM installed. As well, all processes seem to end
normally. With NM installed, 'Killing all processes' fails before an
otherwise clean shutdown, of course this is with unchecked 'available to
all users', otherwise it's unclean.
