The question is not much clear to me, but just thought of mentioning this.
Even when we use IP based throttling, we still can use the same
*org.wso2.carbon.apimgt.gateway.handlers.throttling.APIThrottleHandler*
handler.

There is a section in the link you have provided describing IP level
throttling.
Is it you are looking for or did n't I understand your email correctly.
