Sorry for having wasted your time.  I forgot that I manually change the
fo/pagesetup.xsl file to get this working a couple of months ago.

It would be nice to have this in the customization layer though.

EPUB stylesheets by the transform that creates the -NS distribution
code, so thank you for reporting a bug with a good test case.

At this point I would recommend against using the -NS distribution for
EPUB work.
