Ok, first things first, yes I have owner rights to the Org Forms Library,
second I started the whole thing by opening up the form, and taking a name
out of the To: line, and adding a name.  I go to publish the form.  Some
times just publish form, sometimes publish forms as.  When I go back into
the Org Forms Library and open the fixed form, it opens up without the
changes that I had made.

you replaced the Frmcache.dat file with a known good one? (Sometimes

Can you try just saving the form as an OFT and delete the forms from the org
forms library?  Then if you have some items based on the form, try to open
them, if they open with the old form, it is still in the cache or is
published on the folder or personal forms library (I know you've cleared
that one but I've added it to the instructions anyway.)

The only other place I would check is to see if it was published somewhere
for replies only it actually then marks the form as hidden but Outlook
still reads them.

Try opening the form in design mode by going into Tools\Forms\Design a Form
and select the form, then go into the properties of the To: field Value tab
and see if there's an initial value for that field.  If there is, change it
to what you want it to be and then try Tools\Forms\Publish Form As.

Then you're not doing it right. Put the version number in the name of the
form. Then change the version number in the name of the form.

Changing the version number does not change the name of the form.

It also makes a brand new form so you don't have issues of someone with a
form of the same name but earlier version munging things up. Change the name
of the form, preferably in the version number so you have a change what
makes sense, and your problem evaporates.

Version number is for documentation only and does nothing else.

Make sure you increment the version number (on the properties tab) every
time you make a modification.

So then you have the old form and the new form in the forms library?  That
would be confusing, but if you delete the old form then all the messages
that were created with it would no longer display.  I guess you could hide
it but then you have all these old hidden forms hanging around.

You probably have the earlier version published in your Personal Forms
Library.  Use the Manage Forms in Advanced Options and remove it from there.
Outlook will always look at your Personal Forms Library first.  

If that's not the case, try clearing your forms cache.

Have you confirmed that this is only a problem on your workstation? Have
you replaced the Frmcache.dat file with a known good one? (Sometimes
clearing the cache does not cut it.)

Well, did that help or not?  Also, when you're publishing make sure you do
Forms\Publish Forms As and browse to Organizational Forms Library as opposed
to clicking on the tempting Publish Form button.

Changing the version number does not change the name of the form.

It also makes a brand new form so you don't have issues of someone with a
form of the same name but earlier version munging things up. Change the name
of the form, preferably in the version number so you have a change what
makes sense, and your problem evaporates.

Had the same problem here (forms cache corruption).  Ended up touching each
workstation by deleting the forms cache folder on local pc, then opening up
outlook contacts (do NOT open an actual contact yet), click on tools, forms,
choose form (to apply to the current contact list database), organizational
forms (or wherever your form is) and double click that.  You will see the
form download quickly to your PC and that does it.

I followed the procedures for clearing my forms cache, but when I opened
the
form after again changing it.  It was still the same as before the
change.

constantly updating, but for some reason everytime I make a change and

subsequently re-publish the form, the next time I open up the form it is
the
same as before I made any changes.  It has happened several times now,
and
there doesn't seem to be any rhyme or reason in which it would do this.
Does anyone have any suggestions on why this would happen.

Stupid question: you do have Owner rights to Org Forms Library, right?

When you say the same names keep repopulating the form, which fields does it
repopulate?   How does the form repopulate those fields?  Is there a script
that runs?

No it did not help.  I have tried that also.  but the same names keep

Well, did that help or not?  Also, when you're publishing make sure you do
Forms\Publish Forms As and browse to Organizational Forms Library as opposed
to clicking on the tempting Publish Form button.

Have you confirmed that this is only a problem on your workstation? Have
you replaced the Frmcache.dat file with a known good one? (Sometimes
clearing the cache does not cut it.)
