How big are those attachments?

Hell, even on the LAN at the office, if someone sends me something with
12MB attachment, it still takes a good 30 seconds to open it.

  A common cause of such problems is broken name resolution.  What kind of
connection is running over the ISDN line?  Direct dial-in to your network?  
VPN?  Something else?

Yes it is normal over a line of that speed.
Best solution would be to setup the user with Offline Folders (OST) file.
