The symptoms are "Operation not permitted" in response to Exim's attempt to chown() freshly created files. Normally this chown() is not necessary, as Exim runs as : and created files implictly belong to this user:group. But in some scenarios (sgid bit on a directory) the ownership of a created file might differ from Exim's expectation.
