autonumbering in book

I'm a bit rusty with my FM skills, and having trouble with a very simple issue.

How do I make sure that my figure and table numbers run all the way through a 
multiple chapter book, rather than restarting in each section?

Please respond directly; I don't get the digest.

extra pages

I've been using FM for nearly ten years, but recently had to relearn it
after a couple of years away from it. We are using Frame 7.

 

One of my documents has several extra pages at the end that I can't get
rid of. There seems to be an extra end-of-flow symbol floating near the
end. There is only one flow in the document.

 

I know I have dealt with this before, but my brain can't reach back far
enough to find the answer. Any help would be appreciated!

Flakey cross references

I'm using FrameMaker 7.0 on XP Professional. I've got a book with about
11 chapters in it.

 

I continue to get unresolved cross reference messages. I have gone in
and re-done all of them, since the original files were copied from
another manual, but when I open the documents again, I get the messages
again. None of the files have been moved. I have been printing .ps files
in order to create PDFs of the book. (Somehow that seems to have
something to do with the problem.) The links in the PDF files are fine,
and the page numbers are correct. 

 

Any idea what's happening here?
