I'd start with the seg fault. Check dmesg and/or /var/log/messages to 
see what is dropping core and debug from there.


If it is ns-slapd that is crashing, see

You will need to do

# debuginfo-install ipa-server slapi-nis



The can't contact LDAP server may be another representation of the 
same problem.
