Boot from a CD or other bootable disk.
Go to System Folder>Preferences folder.
Delete the Multiple Users preferences file.
Reboot.

If it's OS X, boot an install CD by holding the "C" key at startup,  
and then select "Reset Password" from the  installer menu.
