Works for me in both 3.10 on Mac OS Catalina and 3.904 (test version) on
Win 10 Pro.  Version 2.6.19 huh well that's pretty dated now I guess :-)

Re point 1 - if you also go to Preferences >> Register >> Layout and check
the "Future transactions after blank transaction" checkbox on it seems to
move the blank transaction to the top :-)

Further to 2: assuming GnuCash version 3.x, read

CSS to modify thre appearance. 

There are also numerous discussions on the lists over the years that cover 
this. I'd try searching for discussions about css.

(Including the blank fields for the "next" transaction)
