  block: relax check on sg gap
  If the last bvec of the 1st bio and the 1st bvec of the next
  bio are physically contigious, and the latter can be merged
  to last segment of the 1st bio, we should think they don't
  violate sg gap(or virt boundary) limit.

  Both Vitaly and Dexuan reported lots of unmergeable small bios
  are observed when running mkfs on Hyper-V virtual storage, and
  performance becomes quite low. This patch fixes that performance
  issue.

  The same issue should exist on NVMe, since it sets virt boundary too.

Bug description:
  Today I did our first production 14.04 to 16.04 LTS server upgrade. I
  had done so a couple of times successfully on test server machines.
  Upon booting up on the 16.04 system/kernel, after the Grub screen the
  server console screen is completely blank.

  Someone suggested using Ctrl-Alt-F1/8, and indeed pressing those
  hotkeys does toggle through the tty screens.

  There is no boot process logged to the server console screen.

  I did have the last 14.04 kernel yet installed, so selected that one
  from the Grub menu. Booting that 14.04 kernel, then server console
  display behaves normally.

  So I suspected perhaps a bum install of the 16.04 kernel. I used the
  following commands while booted to the 14.04 kernel to reinstall the
  16.04 kernel:

  $ sudo dpkg -P linux-image-4.4.0-59-generic 
linux-image-extra-4.4.0-59-generic linux-image-generic
  $ sudo apt-get install linux-image-generic

  Same results booting the 16.04 kernel after those steps.

  Hardware spec is an Intel Atom D945GCLF2D boxed motherboard with Intel
  Atom 330 Dual-Core processor and 2GB RAM. RAID is 3Ware SATA RAID.

  This has been fixed in Ubuntu 16.10 (Yakkety Yak) via

Title:
  Hang after upgrade to 16.04

Bug description:
  Last week I upgraded from 12.04 LTS to 14.04 LTS and then immediately
  to 16.04 LTS.

  12.04 was not entirely stable; something was crashing regularly, and
  the Ubuntu tools make it hard for a user to determine what. The
  upgrade went moderately well; I now get error messages during system
  startup (about an unnamed file not being found) and a couple of other
  bits of flakiness, but I counted it as a success and the system as
  functional.

  This morning I tried to wake up my screen, and nothing much happened.
  I then attempted to ssh to the ubuntu box from another system. This
  requested my password almost instantly, as normal - but then nothing
  else happened, and the connection eventually dropped.

  I conclude that IP and TCP are functional, and it's possible for some
  processes to respond, but not many. So it's not a complete kernel
  hang. (In particular, I'm seeing evidence that it's getting beyond
  things done at interrupt level.)

  I don't have any debugging aids installed, so I don't believe I can
  get a kernel crash dump, which is what I'd want if I were debugging
  this. I *can* potentially retrieve and attach logs, but you'll have to
  tell me which ones are relevant, and do so before they rollover.
  (Also, logging will have to be functioning; IIRC, there were syslog
  issues in 12.04, and while I'd implemented whatever fix was
  reccommended at the time, I haven't looked at my logs since the
  upgrade.)

  This is a desk top system originally from System 76 - i.e. built for
  linux - that's also running a bunch of server software (postfix,
  apache, ...) I was not (knowingly) running anythign unusual at the
  time - probably Unity, a few shells, firefox, maybe guncash and/or
  emacs - and all the usual demons.

  IIRC, I was not at the very latest versions of all software installed
  - some new versions ahd come out since I upgraded, and I was going to
  deal with installing them on the weekend.

  I'm going to hard reboot the system now. I can then gather identifying info. 
If I have time this AM before work, I'll check for standard things you want in 
all bugs, and add them. (Right now I'm posting from my Mac laptop ;-))

  [Impact]

   * If using the bind9-resolvconf service to have the local named
  managed resolv.conf, the service exits after running starting, and the
  system resolv.conf ends up reverting to the default content.

   * The user is effectively prevented from using bind9-resolvconf to
  manage their local resolv.conf.

   * The issue is that the bind9-resolvconf service needs to detected as
  still running even after the /etc/resolv.conf modification occurs. As
  per Debian Bug 744304: "RemainAfterExit tells systemd that a service
  should be considered running even after it exited. Currently, systemd
  thinks the service went inactive after the ExecStart command exits,
  and then immediately calls the ExecStop command, thus removing
  127.0.0.1 from resolvconf."

  [Test Case]

   * Install bind9-resolvconf with a local bind9 configuration. Start
  the bind9-resolvconf service and the prior content of /etc/resolv.conf
  will remain even if it differs from bind9's configuration.

  [Regression Potential]

   * I believe the regression potential to be very low for this change.
  The bind9-resolvconf service currently does not work as expected.
  Users may have made manual changes locally, as suggested in this bug,
  but those seem to generally not be permanent solutions and should not
  collide with the change to the service.

  ---

  I enabled the bind9-resolvconf service and restarted my system,
  because I want to use the named running on localhost as my nameserver.

  Even after the restart, however, the nameservers in /etc/resolv.conf
  (actually /var/run/resolvconf/resolv.conf) were still the ones
  provided by DHCP. This, despite the fact that the logs claim that
  bind9-resolvconf ran successfully during boot.

  I tried manually running "sudo systemctl start bind9-resolv.conf", and
  again, the logs claim it ran, but /etc/resolv.conf was unmodified.

  Finally, I manually ran "sudo /bin/sh -c 'echo nameserver 127.0.0.1 |
  /sbin/resolvconf -a lo.named'", i.e., the command listed in
  /lib/systemd/system/bind9-resolv.conf.service, and _that_ successfully
  updated /etc/resolv.conf.

  After doing that, interestingly, "sudo systemctl stop
  bind9-resolv.conf" _also_ doesn't change /etc/resolv.conf, i.e., it
  still retains the 127.0.0.1 line which I added by running the
  resolvconf command manually.

actually listens on; workaround via configuration file but needs fixing.

  sudo mkdir -p /srv/backup
  sudo xtrabackup --backup --target-dir=/srv/backup

  Backup fails to connect to mysql with the error in the original bug
  report

  [Regression Potential]
  Limited; we missed this and two other compilation flag changes during the 
original update - the proposed updates re-sync the Ubuntu packaging with the 
upstream packaging in terms of compilation flags and value.

  percona-xtrabackup was upgraded from 2.2.3-2.1build1.1 to 
2.3.7-0ubuntu0.16.04.1.

  It appears that MySQL default socket file path has changed in the new
  version.

  After the update my backups started failing with the following:

  170402 06:30:04 Connecting to MySQL server host: localhost, user: root, 
password: not set, port: not set, socket: not set
  Failed to connect to MySQL server: Can't connect to local MySQL server 
through socket '/tmp/mysql.sock' (2).

  Workaround:

  Add the following in /root/.my.cnf or similar:

  Additional comments:

  1. There seems to be a circular dependency between nfs-mountd and 
nfs-kernel-server
  2. I can get it working by changing the AFter,Requires in 
/lib/ssystemd/system/nfs-{mountd|server}.service files. I have managed to get 
nfs-kernel-server to start but not nfs-mountd.
  3. /usr/lib/systemd/scripts/nfs-utils_env.sh references 
/etc/sysconfig/nfs which is Centos/RedHat location of this file. Also 
/etc/default/nfs does not exist. (possibly unrelated to this bug)
  4. A file "/lib/systemd/system/-.slice" exists. this file prevents 
execution of 'ls *' or 'grep xxx *' commands in that directory. I am unsure 
whether this is intended by the systemd developers but it is unfriendly when 
investigating this bug.

  Attempted solution:

  [Impact]

   * rabbitmq-server can segfault along codepath which happens to "open
  a port with the same fd multiple times".  Doing so is undefined (and
  unsafe in erlang, though segfaulting is unintentinal).

   * This only happens on specific versions of erlang, but the rabbitmq-
  server code is agreeably incorrect per erlang and has been fixed
  upstream.

  * This only affects xenial & yakkety.

  * The codepath belongs to an internal helper for writing to stderr,
  this prevents useful diagnostic information from being provided to a
  user.

  [Test Case]

   * Make sure your hostname resolves to something unreachable, I've
  selected 192.168.2.2, install rabbitmq-server, witness segfault.

   * # hostname blah
   * # echo "192.168.2.22 blah" >> /etc/hosts

   * # ping blah
  PING blah (192.168.122.2) 56(84) bytes of data.

format_stderr() helper function.

   * Limited to diagnostic messages path, so its really only seen when
  something is configured incorrectly.  That being said, any execution
  through this path today will segfault and without any diagnostic
  information to figure out what, so seems infinitely better.

   * This fix from upstream has been in place over a year without any
  issue, and was originally code that was working around buggy/flaking
  erlang library that has (according to upstream reports) been fixed
  since erlang 17, thus uneeded.

   * While the rabbitmq-server in trusty has this offending code, the version 
of erlang does not segfault.  Additionally, the fix provided by upstream is not 
necessarily sufficient on erlang < 17 that is in trusty, so I have not fixed it 
there.

  Starting rabbitmq-server triggers segfault.
  The segfault happens when the host is not reachable, for instance, which 
breaks the installation of rabbitmq-server package.
  It is comprehensible that an error must occur, but segfault should not be a 
default behaviour.
  This has been tested on 16.04 and 16.10, archs ppc64el and x86_64

   * The fix was NMU'd in Debian in the version immediately after
  16.10's. I believe the regression potential is very low as this is a
  clear bug-fix from upstream.

Currently libseccomp version in Ubuntu are:

 libseccomp | 2.2.3-3ubuntu3   | xenial | source
 libseccomp | 2.3.1-2ubuntu2   | yakkety| source
 libseccomp | 2.3.1-2.1ubuntu1 | zesty  | source

The difference between 2.2.3 and 2.3.1 is 63 upstream commits.

Of those commits, 7 are already cherrypicked into xenial for s390x
support.

However that s390x support is incomplete as multiplexed syscalls are not
supported.

A request has been filed to support multiplexed syscalls in libseccomp
in xenial at
https://bugs.launchpad.net/ubuntu/+source/libseccomp/+bug/1679691

That is a request for further 18 commits to backport, bringing the total
to 25.

Looking at the remaining 38 commits there are:
- documentation updates
- tools updates
- tests updates
- bugfixes
- updates to syscall tables for linux 4.3, 4.5-rc4+

IMHO, in the future when libseccomp is updated to support 4.10 kernel
syscalls, it should be backported back to xenial too, to properly suppor
the HWE kernels.

  Unity panel service crashes (removing indicators from panel) when calendar 
menu is opened and there are some key presses.

  [Test Case]
  This is a quite random bug that is not easy to reproduce, it happens 
sometimes that you open the indicator-datetime and after a keypress the panel 
crashes.

  [Regression Potential]
  Nothing expected, but calendar item in datetime might behave differently on 
key-presses.
  Although the fix is quite safe since we're just ensuring that we disconnect 
from parent widget signals on menuitem destruction.
