Bug description:
  [Impact]
  If a dkms package is installed which has REMAKE_INITRD or the same setting 
has been manually configured by a user then when a kernel is removed its 
possible for an ".old-dkms" file to be left in /boot with no associated kernel.

  bug 1515513 dealt with removing old-dkms initrd files using the
  kernel's prerm hook, but that is only executed for the kernel version
  being removed: any other old-dkms file generated prior to that would
  not be removed by the hook, taking space in the /boot directory and
  being carried forward with every upgrade.

  Note: Filling up the /boot partition causes updates and upgrades to
  fail.

  [Test Case]
  As the fix for bug 1515513 is available on Xenial and Bionic it is no longer 
possible to reproduce this by simply installing and updating kernels - dkms 
2.2.0.3-2ubuntu11.3/xenial or 2.3-3ubuntu1/bionic would be required for that. 
In order to replicate it an old dkms file will be created by hand.

  This assumes a new Xenial/Bionic schroot.

  1) create files to work as a placeholders for old dkms files (there are 4 
possible namings for these files)
  touch /boot/initrd-4.0.0-0-generic.img.old-dkms 
/boot/initramfs-4.0.0-0-generic.img.old-dkms 
/boot/initrd.img-4.0.0-0-generic.old-dkms /boot/initrd-4.0.0-0-generic.old-dkms

  2) install 3 old kernels, r8168-dkms, and the current initramfs-tools
  * xenial:
  apt-get install -y linux-image-4.4.0-21-generic linux-image-4.4.0-22-generic 
linux-image-4.4.0-24-generic r8168-dkms initramfs-tools=0.122ubuntu8.12
  * bionic:
  apt-get install -y linux-image-4.15.0-32-generic 
linux-image-4.15.0-33-generic linux-image-4.15.0-34-generic r8168-dkms 
initramfs-tools=0.130ubuntu3.3

  3) install the headers for the old kernels (forces dkms to run)
  * xenial:
  apt-get install -y linux-headers-4.4.0-21-generic 
linux-headers-4.4.0-22-generic linux-headers-4.4.0-24-generic
  * bionic:
  apt-get install -y linux-headers-4.15.0-32-generic 
linux-headers-4.15.0-33-generic linux-headers-4.15.0-34-generic

  4) verify that there are 7 old-dkms, the 4 manually created ones and one for 
each installed kernel
  ls -1 /boot/*.old-dkms

  5) install the initramfs-tools from proposed that contains this fix
  apt-get install -y initramfs-tools

  6) verify that the manually created old-dkms files were removed and that 
there are only 3 files now, one for each installed kernel
  ls /boot/*.old-dkms

  7) mark kernel images and headers as automatically installed
  apt-mark auto linux-image-4*-generic linux-headers-4*-generic

  8) autoremove the older kernel
  apt-get autoremove -y

  9) verify that there are now only 2 old-dkms, one for each installed kernel
  ls -1 /boot/*.old-dkms

  
  These steps guarantees that:
  - orphaned old-dkms are correctly removed from /boot
  - non-orphaned old-dkms are kept
  - when kernel is removed the related old-dkms are also deleted

  [Regression Potential]
  Somebody out there might expect the .old-dkms file to be kept, but that seems 
like an odd expectation.

  One notices *.old-dkms files being left behind still sitting on the
  disk after purging the related kernel. This can cause /boot to become
  full, and when it gets really bad, even sudo apt-get autoremove won't
  fix the problem - only deleting the old-dkms files manually solves the
  problem.

   * When logind is not available, shutdown command fails to schedule a

  The expectation is that system goes to shutdown.

  It is buggy if the system remains up - i.e. command returns to shell

  Used Landscape (Paid Canonical Subscription) to upgrade one of my machines.

  Landscape only shows "In Progress" for more than 8 hours now and asked
  for a reboot of the machine in a second alert.

  In the reboot attempt I get the message:

  Steps to reproduce:
  * Fully updated 14.04.5 machine
  * Open Landscape
  * Choose the machine
  * Choose Packages
  * This computer can be upgraded to a newer release
  * Apply
  * Wait 2 hours
  * Alert comes in a seperate Landscape message Machine is ready for reboot
  * Choose Info... Power
  * Deliver to selected computers as soon as possible
  * Error message

  [Impact]
  I have a UEFI-based server with both a graphical head and a serial console 
(Cavium Sabre). If I boot the netboot (d-i) installer, the install menu appears 
on both, and I'm able to interact with the installer over both the serial 
console and the graphical head. However, when booting into the *installed* 
system (w/ HIDDEN_TIMEOUT disabled), the GRUB menu only appears over the 
graphical head. I can use either serial or graphical for input - that is, if I 
monitor the graphical head I can see the changes I'm typing on the serial 
console, but the serial console remains blank until I boot into Linux.

  Note: MAAS installations are not impacted by this because curtin adds
  an /etc/default/grub.d stub that manually sets
  "GRUB_TERMINAL=console".

  [Test Case]
  Install the system using d-i over the serial console.
  Comment out the HIDDEN variables in /etc/default/grub.
  sudo update-grub
  Reboot - and watch for the GRUB menu over the serial console.

  Rebuilding debian-installer will be necessary in order to get a new
  mini.iso (Minimal ISO image of Ubuntu) containing this particular fix.

  Despite the fact that a full ISO are no longer built for some of the
  affected stable releases, one can use what is called a minimal ISO
  image (mini.iso) of Ubuntu. This mini.iso uses a text-based installer,
  resulting in a smaller image. The minimal ISO image will download
  packages from online archives at installation time instead of
  providing them on the install media itself.

  [Impact]

  This is the same issue reported in 
https://bugs.launchpad.net/ubuntu/+source/preseed/+bug/1452202
  However that is marked as 'Fix Released' and the issue is still occurring so 
it seems best to just open a new bug according to Eric's comment.

  The present hostname preseed regex looks like:

  if ! echo "$RET" | grep -q 'ubuntu'; then
     ...
     /bin/sed -i "s/$CURRENT_HOSTNAME/$NETCFG_HOSTNAME/" /etc/hosts
     ...
  fi

  There are still multiple problems with them according to Peter's
  comment
  (https://bugs.launchpad.net/ubuntu/+source/preseed/+bug/1452202/comments/64)
  and Mathieu's comment
  (https://bugs.launchpad.net/ubuntu/+source/preseed/+bug/1452202/comments/93).

  - The default hostname is 'ubuntu', and is the value you will get if
  nothing is preseeded -- we *do* need to skip it in this case, although
  the grep may need to be more precise. A name like 'ubuntuxenial'
  should match this grep.

  - The sed needs to be fixed to more appropriately catch for the exact
  string, rather than any substring -- ie. it should not match
  'ipv6-allnodes' if the name received from DHCP was 'node'. It should
  only match and replace for 'node'.

  So the fix will try to:

  1) Keep the if statement and add the -w option to grep to be more robust
  2) Modify the sed cmd with word boundaries (\b) to be more robust as well.

  [Test Case]

  Install Ubuntu using a preseed[1] file[2] and add the following line in the 
preseed recipe :
  d-i netcfg/hostname string 

  Change the HOSTNAME to see if the hostname in /etc/hosts can be
  updated correctly.

  In other words, we simply make it more robust to select only pattern
  containing matches that form whole word... catching 'ubuntu' hostname
  as-is but not catching pattern where ubuntu is part of the user chosen
  hostname as follow :

  "sometextubuntusometext"
  "ubuntusometext"
  ...
