Haven't had a response to this open position ... so I'll try one more

We have a histo tech that is retiring in December, and we need to fill
that position. We are a small lab that services a 100 bed hospital and
the surrounding physicians. We are located in California at the base of
the Sierra Foothills. If you are interested or know someone who may be

[Histonet] GMS

light green is only staining the outer edges of the specimen. Any Ideas?

[Histonet] Cytology specimens

I would like to poll the Histo group concerning cytology specimens. The
discussion has come up about when to discard a fluid after processing,
and fluids that have no orders. Also, because we are a contracted
service for the hospital, and are not connected to their LIS. Cytology's
that are ordered do produce a requisition that sometimes does not follow
the specimen, which leads to missed processing. How are others handling
these two situations.

[Histonet] Cryostat Chirp

[Histonet] Specimen release

We are revisiting our policy of releasing tissue to patients. Presently
we release placentas' to patients for Religious reasons, but it seems
to be escalating to include appendix, uterus, etc. I would like to

[Histonet] Telepathology

We are being asked to provide evaluation of EUS (Endoscopic Ultra Sound)
specimen from  a remote location. We are provided with a microscope and
a laptop, does anyone know of a simple way to capture and transmit
images back to the pathology lab?

[Histonet] Requisitions

We are having a discussion as to whether the requisition needs to remain
with the specimen at the grossing stage or can it be in a separate stack
on the grossing table, while the actual specimen is left in the on deck
tray.   I believe I have seen something that speaks to this through the
CAP guidelines, but am unable to find it.  Any help would be
appreciated.

[Histonet] Sections

One of our pathologist has requested a change in the way we cut GI
specimens.  We presently put three levels on one slide. Exp.  we face
the block, cut a ribbon and take one section of that ribbon and catch on
the slide. We then progress into the block and repeat the process until
we have three levels on the slide.  The new suggestion is to simply face
the block, create a ribbon, and take three to four section of that
ribbon and that's it no more progression into the block.  The thinking
is that the Pathologist will see what they need to in those levels, and
if not they will call for deepers. I have never cut like this and just
wanted to hear some thoughts on this method.

[Histonet] Breast processing

We have someone come in over the weekend and embed the tissue.  If we go over 
the time protocol for some reason, which is not often, we will evaluate the 
specimen by FISH. Once you take the bitter pill of someone coming over the 
weekend, it works very well.

Can you please give me your routine for processing breast cases over the 
weekend. Do you extend the time in alcohol to achieve the 48 hour maximum, call 
someone in over the weekend, or just indicate on the patient report that the 
fixation extended beyond the 48 hours limit for HER 2, 72 hours for ER and PR. 
Is the ischemic time recorded within the lab as well as on the patient report?

[Histonet] Cassette Protocol

It seems to me that this is somewhat guided by the funds you have to
spend on a cassette labeler.  Otherwise, if you label by hand, no
problem.  Also, the ordering factor can become a bit more detailed if
you have a varied protocol. We keep it pretty simple; white being for
single levels, greens for multiple levels, and yellow's for cytology.
We do depend on our work list and the histologist skill to identify
specimens and their possible special handling (IE; mamatome breast
biopsy's = 5 levels).

We also transport smears from the clinical lab to our Pathologist and
transport them back.  The Hematology techs put the Pathologist's
comments into our computer system for review by the clinicians.

We too are a small lab in California, and faced these same questions.  We 
simply document all this in the report.  For example; we know that our tissue 
comes out of formalin on the processor at 2330.  We have the surgery staff 
document the time the specimen went into formalin.  We then subtract that time 
form 2330 and have the hours in formalin.  You are correct that it took a great 
deal of effort to see that surgery complied, but after they realized we were 
relentless with phone calls, things fell into place. Oh yes! On the weekend 
someone must come in to remove the tissue from the processor.

Robin this is the protocol we use to showing new bone old bone.

Procedure for Eosin Y 0.6 % For Staining Bone Specimen
Chemicals
EosinY, Sigma E-4382,
100% Ethanol,
Phloxine B (Sigma P-4030)
Orange G(sodium salt-Sigma O-1625)
Preparation:
Stock 0.6% Eosin
Add 6g of eosin to 900ml 100% Ethanol and100 ml of H20; stir to dissolve.
Add 50 ml glacial acetic acid adjust PH to 4.6 and 5.0. The color of the 
solution will change from opaque green to clear red.
Stock 1% Phloxine B Solution:
Dissolve 1g of Phloxine B in 100 ml distilled water and filter to make a 1% 
solution.
Stock 2% Orange G Solution:
Dissolve 2 g of Orange G in 100ml distilled water and filter to make a 2% 
solution.
Working Solution Eosin 0.6%
Make a working solution by adding 6 ml of 1% Phloxine and 6 ml of 2% Orange G 
to 238 ml of 0.6% eosin.
Old bone dark orange.
New bone light orange.
Stain 15 to 30 seconds depending on what intensity you prefer.

Thank you for your response.  CAP is exactly why I am asking this
question.  WE do presently have procedures for the exceptions you
mentioned.  However, it came up in conversation that shavings may be
considered infectious waste.  I have yet to find anything in the CAP
information that mention this material.  I did note your mention of
block disposal, and we have that under control per CAP requirements.
It's the grey area of shaving we are questioning.

You may receive many opinions on this --- just a heads up.  Many
clinical labs do not consider shavings or debris from cutting to be red
bag (hazardous) waste as they are no longer a 'biological hazard' due to
the processing process which incorporates heat, alcohol and a fixative
that is expected to effectively kill any infectious agents. There are
some exceptions, which you can find in the CDC or OSHA guidelines such
as dealing with tissue of brain (Jakob-creutzfeldt) lung (TB or other
air-borne pathogens).  CAP also has requirements/guidelines for handling
these cases which you will need to have documentation for the processing
procedure.  I would suggest that you refer to these guidelines and also
to your institutions regulations on what is regarded as hazardous waste.
Above and beyond anything else make sure you document and site in your
procedures manual your sources.

[Histonet] Pap sign out

We have always had our Pathologist sign out all Pap results, and are now
considering having out Cytotech sign out Pap's. Is there an agency we
have to report o before we start this process. Also, how are others
choosing the 10% for review by the Pathologist?

[Histonet] Bone Marrow clot

We have recently been receiving bone marrow specimens where the syringe
has been heparinized before the aspiration is retrieved.  We are having
problem getting the clot sections to stay on the slide.  Could the
heparinization be causing this problem, and if so, is there a solution?

[Histonet] !0% buffered formalin corrosive?

I have a question around 10% buffered formalin being shipped to us.  The
shipper considers this corrosive.  I have been told that if the formalin
is shipped in 6oml biopsy bottles it does not receive that corrosive
rating when shipped.  I have also been told that if the shipment of
formalin is on a pallet and it weights 220 lbs or more, it will not
receive the corrosive rating when shipped.  Can someone bring some
clarity to the corrosive rating for me.

As an old carpenter I quickly adapted a speed square to do pretty much
the same thing as the $775.00 unit or the torpedo level.  The speed
square has the advantage of squaring the block holder to a true flat
surface (the base of the microtome) it works quite well ... it is
positive and quick.  See the Ace website below. I also use the torpedo
level as a double check.

have a longer handle and don't seem quite as hot as the square Histo

If you have clicked on the aa aa attachment and can't get out of the loop ... 
use the ALT control delete buttons pushed simultaneously and you will get to 
the task manager ... highlight the attachment and click the button that says

[Histonet] Frozen section

2009-03-25 Thread Martin, Gary
We need to make a change in the way we presently account for our frozen
section while doing them.  Presently we receive the requisition with the
first specimen, then pathology is responsible to account for any
subsequent specimens.  The problem is that the subsequent specimens are
typically labeled poorly, and we are trying very hard to conform to the
CAP guidelines. So ... when the specimens are not labeled in detail, it
requires follow up calls to gain the proper information.  I would like
to know how other facilities are handling multiple frozen sections.

I try to embed so as to have a minimal amount of paraffin to scrape from the 
blocks. ;)

But, I do scrape using the handle end of the same forceps I use to pick up the 
ribbon and tease the sections. No sharp edge. No electricity.

First ... even if the local authorities allow this ... it doesn't make Green 
sense to do it, especially when there are other very workable alternatives.  
The path we chose was to purchase a very simple gravity feed recycler produced 
and sold by Creative Waste Solutions (888) 795-8300. Check them out.

We have a question regarding the disposal of formaldehyde. We were told at our 
hospital that a consultant said it was okay to dump formaldehyde down the 
drain. I believe they said it was okay to dump 15 gallons or so a day! We are 
not to fond of this idea and would like to know what everyone else is doing. 
How is everyone disposing of their formaldehyde? We would be especially 
interested in what other hospitals in CT are doing.

My wife, the best section cutter in the world worked right up to the
bitter end of her pregnancy with our third daughter, dropped an old

Besides a few little nicks that we all get, about 10 years ago after
having worked in histology for almost 25 years I was finally in a lab
with windows!  Early one morning as I was sitting there cutting with my
back to the window, dawn just breaking.  A head popped up outside the
window and the tech facing the window screamed just as I was running my
thumb over the block.  I jumped; the chuck came down and sliced into the
pad of my thumb down to the bone. I ran to the sink holding my thumb and
ran it under water.  It's pretty gross to see the bone in your thumb!
Funny thing, there was no pain until the ER doc filled it full of the
stuff to numb it.  Got lots of stitches and even now it sometimes hurts.

I've heard that tale, but didn't know who did it! 

I had a coworked who cut the pad off his thumb. We went to the ER, where
I learned he had put it in the trash. I went back and dug through the
paraffin scraps and got it - washed it in saline and Betadine.. The ER
dr glued it back on and he has a thumbprint again! 

When I caught myself on fire, cleaning with solvants too close to the
hot plate (the fumes ignited), he was the same guy who yelled, Get the
marshmallows - the supervisor's on fire!. I still get a good laugh
about that! J:)

And it's only Tues!! Trick or treat!!!

Not me, but the guy who trained me.  Remember the old knife sharpeners
that looked like record players?  He was removing the blade (they got
pretty oily) and it slipped.  His natural reaction was to grab for it,
slicing three fingers to the bone.  Made for a good cautionary tale...

 Me - 1982 - working in a S. Florida hospital I sliced open the base of
my thumb on the corner of a big steel blade as I was sectioning. Didn't
even notice it until I saw the blood dripping off my elbow.

I went to the ER and was sitting on a gurney with my hand submerged in a
bowl of betadine waiting to be stitched up. After what seemed like a
pretty long time,  I deftly fell off the gurney onto the floor tipping
the entire bowl all over onto myself.

Upon hearing the clamor of me hitting the floor and the metal bowl
banging around a rush of ER staff charged in and I got prompt service
thereafter. I still have the scar, but I never cut myself again :-)
